This notice explains the contact form on the Derniva website.
What the form collects
Your first name, country of residence, preferred language and either your email address or telephone number. We also record your contact preference, acknowledgement of this notice and a request reference. If you choose a treatment category you are interested in, we also save that optional preference to help direct your enquiry.
What your enquiry is for
Your details are used to record and follow up your request for contact. Submitting this form is not a medical assessment, a confirmed appointment or a treatment agreement. Please do not submit diagnoses, photographs or medical records.
Storage and access
Enquiries are stored in a protected database. They are not displayed publicly. The form does not forward your details to advertising services. A separate agreed process is needed if a clinical team requires medical information.
Your choices
For questions about your details or to request access, correction or deletion, contact info@derniva.com. You can choose to contact us directly instead of using this form.
Enquiry notifications
Your submitted details are sent through Resend to the Derniva team's Gmail inbox so the team can respond to your enquiry. A copy of the notification and its sending status are retained in the protected database.
External services
WhatsApp and external links open third-party services with their own privacy terms. The centre’s video is served by this site and plays when you choose to watch it. Optional advertising measurement is explained below.
Your privacy choices
We load Google Ads measurement only with your permission. Google may receive the visited page address and title, ad click and cookie identifiers, IP address and generic saved-enquiry, WhatsApp-click or phone-click events. Your name, email, phone number and treatment preferences from the form are not sent to Google. We do not use ad personalisation, remarketing or enhanced conversions. Decline or withdraw permission at any time through Cookie preferences. Your choice is stored on this device for 180 days; clearing browser data resets it. The form works regardless of this choice.
