This notice explains the contact form on the Derniva website.
What the form collects
Your first name, country of residence, preferred language and either your email address or telephone number. We also record your contact preference, acknowledgement of this notice and a request reference. If you choose a treatment category you are interested in, we also save that optional preference to help direct your enquiry.
What your enquiry is for
Your details are used to record and follow up your request for contact. Submitting this form is not a medical assessment, a confirmed appointment or a treatment agreement. Please do not submit diagnoses, photographs or medical records.
Storage and access
Enquiries are stored in a protected database. They are not displayed publicly. The form does not forward your details to advertising services. A separate agreed process is needed if a clinical team requires medical information.
Your choices
For questions about your details or to request access, correction or deletion, contact info@derniva.com. You can choose to contact us directly instead of using this form.
Enquiry notifications
Your submitted details are sent through Resend to the Derniva team's Gmail inbox so the team can respond to your enquiry. A copy of the notification and its sending status are retained in the protected database.
External services
WhatsApp and external source links open third-party services with their own privacy terms. The centre’s video is served by this site and plays when you choose to watch it. This site does not use advertising or analytics tracking.
